Valenz Health¹ and its affiliates (“Valenz,” “we,” “our,” or “us”) respect your privacy and are committed to safeguarding your personal information. This Privacy Notice explains how we collect, use, disclose, and protect personal information when you use our websites, mobile applications, and services (collectively, the “Services”).

This Notice also describes your rights under applicable privacy laws, including the Health  Insurance Portability and Accountability Act (“HIPAA”), the California Consumer Privacy Act as amended by the California Privacy Rights Act (“CCPA/CPRA”), and other state consumer privacy laws.

 

Information We Collect 

We collect personal information to provide and operate our Services. Depending on how  you interact with us, the categories of information we may collect include:

  • Identifiers: Name, mailing address, email address, telephone number, IP address, online identifiers
  • Account Information: Username, password, and other registration data
  • Health Information: Medical provider details, demographic data, and health related information subject to HIPAA
  • Internet and Technical Information: Browser type, device identifiers, operating system, pages visited, time spent on the site, referral URLs, and similar usage data collected through cookies and tracking technologies

 

Children’s Privacy (COPPA) 

Our Services are not directed to children under the age of 13, and we do not knowingly collect personal information from children under 13. If we become aware that we have collected personal information from a child under 13 without verifiable parental consent, we will delete that information promptly.

We do not sell personal information for monetary consideration. However, we may disclose or share personal information as follows:

  • With healthcare providers or payers as needed for care coordination

  • With successor entities in the event of a merger, acquisition, or asset transfer
  • With analytics and advertising partners, including through cookies and tracking  technologies, which may involve sharing identifiers such as IP address and online identifiers for advertising and cross-context behavioral advertising purposes

Some of this sharing may be considered “sharing” under the CPRA, even if no money is exchanged.

All third parties are required to protect personal information consistent with applicable law and contractual obligations.

 

HIPAA and Protected Health Information (PHI)

When Valenz acts as a Business Associate to Covered Entities under HIPAA, we collect, use, and disclose Protected Health Information (“PHI”) only as permitted by HIPAA and applicable agreements, including for:

  • Treatment
  • Payment
  • Healthcare operations
  • Public health and safety activities
  • Legal and regulatory requirements

We are required by law to maintain the privacy and security of PHI and to notify affected individuals in the event of a breach involving unsecured PHI.

 

Your HIPAA Rights 

You have the right to:

  • Access and receive copies of PHI
  • Request amendments
  • Request restrictions
  • Request confidential communications
  • Receive an accounting of disclosures
  • File a complaint with Valenz or the U.S. Department of Health and Human Services

 

State-Specific Privacy Rights 

California Privacy Rights (CCPA/CPRA) 

California residents have the right to:

  • Know what personal information is collected, used, and shared
  • Request deletion of personal information
  • Correct inaccurate personal information
  • Opt out of the sale or sharing of personal information
  • Limit certain uses of sensitive personal information (if applicable)
  • Not be discriminated against for exercising privacy rights

California residents may exercise their rights through our California Privacy Choices link or as described in our California Privacy Rights Notice.

 

Other State Privacy Laws 

Residents of certain states, including Colorado, Virginia, Connecticut, Texas, Oregon, Montana, Utah, Iowa, and Indiana (effective January 1, 2026), may have additional rights depending on state law.

 

Data Retention 

We retain personal information only for as long as reasonably necessary to fulfill the  purposes described in this Notice, unless a longer retention period is required or permitted by law.

 

Data Security 

We maintain administrative, technical, and physical safeguards designed to protect  personal information, including encryption, access controls, employee training, and system monitoring. No system is completely secure.

 

Deletion Requests 

You may request deletion of your personal information by emailing privacy@valenzhealth.com. We will verify your identity and respond in accordance with applicable legal timelines and exceptions.

 

Changes to This Privacy Notice 

We may update this Privacy Notice periodically. Updates will be posted with a revised  effective date. Continued use of the Services constitutes acceptance of the updated Notice.
